Your first visit to ENT for KIDS Alabama is an important time to establish a vital foundation for our relationship with you. To save a significant amount of time at your first visit we strongly encourage you to log on and establish a patient portal account. On the patient portal you will be able to fill out and e-sign the new patient forms in advance of your appointment.

Patient Forms
If you are a new patient to ENT for KIDS Alabama, after you have called and made your initial appointment, and we have your childs information in the system. Please give us your email address. After this has been entered in our system, you will receive an email invitation which will take your to your child's portal. Follow the instructions to establish your account. Please be sure to use your childs legal name as it appears on the insurance information, do not use his shortened name or nickname as the EMR system will not recognize the alternative name. Once in the portal you will see your child's appointment date on the home page. You can update your child's information in the Health History tab (It should be an icon of a heart), and the day before the appointment you will receive information on how to complete online check in through our patient portal app. On the Portal you will also be able to view and confirm your appointment time, and even request an appointment change if needed, and after the appointment you can view details of the visit and the discussed treatment plan. There is also a convenient link to pay towards any balance you may have on your child's account.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. You may educate yourself on your child's symptoms and condition by reviewing the content on this Web site. Please be sure all necessary insurance referrals are sent to our office by your child's primary care physician prior to your visit. When you come in for the visit, please be sure to bring a valid driver's license, insurance card and, if necessary, any legal guardianship papers that may be required in order for us to see and treat your child.

Directions: The office is located in Suite A101 of The Crossroads, an office complex located at 5406 HWY 280, Birmingham 35242 .
The Crossroads is an office complex located at the intersection on HWY 119 and HWY 280.
If heading east on hwy 280 :
Once you pass HWY 119 you may turn right into the CVS parking lot and continue straight through to the office complex. You can also turn right at the shell station, the YMCA, and the red light at the Hugh Daniel Dr intersection. All of these will take you to Hugh Daniel Dr and you can turn right until you reach the office complex, and it will be on your left.
If heading west on hwy 280 :
Take a left onto Hugh Daniel Dr. You will pass Regions bank and the YMCA and our office will be on your left directly behind the shell station and the CVS.
If heading north on hwy 119:
Take a right onto Hugh Daniel Dr directly before the Express Oil Change at the corner of HWY 119 and HWY 280.
If heading south on hwy 119:
Take a left onto HWY 280 while in the rightmost turn lane. Then, you may turn right into the CVS parking lot and continue straight through to the office complex. You can also turn right at the shell station, the YMCA, and the red light at the Hugh Daniel Dr intersection. All of these will take you to Hugh Daniel Dr and you can turn right until you reach the office complex on the left.
