Your first visit to ENT for KIDS Alabama is an important time to establish a vital foundation for our relationship with you.  In this section you will find our practice's guiding principles and you will find all the practical information you need for your visit, such as a map and directions to our office, practice hours, payment policies and more. There’s also background information about our first visit procedures. To save a significant amount of time at your first visit we strongly encourage you to log on and establish a patient portal account.  On the patient portal you will be able to fill out and e-sign the new patient forms in advance of your appointment.

Patient Forms

If you are a new patient to ENT for KIDS Alabama, after you have called and made your initial appointment, and we have your childs information in the system, please log on to the patient portal by clicking on the patient portal tab above.  Follow the instructions to establish your account.  Please be sure to use your childs legal name as it appears on the insurance information, do not use his shortened name or nickname as the EMR system will not recognize the alternative name.  Once in the portal you will see your child's appointment date on the home page.  Click on the appointment date and you will see the link for the online patient medical history form and the two privacy and consent forms that need to be e-signed.  Alternatively you may print these forms and bring them in to the office or you may wait to fill them out when you arrive.  If you do wait to fill them out at the first visit we ask that you come in 15-20 minutes prior to your appointment.  After your first visit you will only need to go back to this section if your childs medical or medication history changes.  On the Portal you will also be able to view and confirm your appointment time, and even request an appointment change if needed, and after the appointment you can view details of the visit and the discussed treatment plan.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. You may educate yourself on your child's symptoms and condition by reviewing the content on this Web site. Directions

Directions:  The office is located in Suite 42 of 280 Village an office complex located at 2807 Greystone Commercial Blvd, Birmingham 35242.   

Greystone Commercial Blvd. is an access road that can be seen on the on the right side just past HWY 119 if heading south on HWY 280.  The access road has several entrances from 280 the first of which is at the Jim N' Nick's Restaurant.  Also located on this access road are the Issis and Son's carpet store and the large yellow building housing Game Room Designs/Shoemaker's Pools.  280 Village is the last complex on this access road before you hit Lee Branch Shopping mall.

If heading south on hwy 280:

Once you pass HWY 119 you may turn right at the entrance of Jim N Nicks Restaurant.  If you miss that turn there is another place to turn just after Jim N Nick's.  Both of these right turns will feed you onto Greystone Commercial Blvd.  You take a left and go to the end of this road where you will see a sign and the entrance to 280 Village.  We are located in Suite 42.   If you miss these turns off 280, just take a right at the light onto Doug Baker Blvd and follow the directions below.

If heading north on hwy 280:

Take a left onto Doug Baker Blvd and then take the first right into the mall and head past the Zaxbys and past the Panera Bread.  Keep going straight out of the mall complex just past the Panera Bread, once out of the mall parking lot, take an immediate left and 280 Village is located directly in front of you. We are located in Suite 42
